Effect of the complex physical therapy on the upper extremity function, functional independence and cognition in post-stroke patients

Motor dysfunction and cognitive impairment (CI) are the most prevalent disabling among stroke consequences. CI decreases effectivity of motor rehabilitation, but itself may also influence manifestations progression CI. So development study novel physical therapy tactics, which aiming to target both these syndromes, becomes a subject great interest nowadays.
 The aim was evaluate impact different approaches on upper extremity function, cognition functional independence in patients 1 year after ischemic stroke.
 Materials methods. Totally there were 72 examined 1-year period first-ever anterior circulation stroke. Neurological status, function assessed with National Institutes Health Stroke Scale (NIHSS), Fugl-Meyer assessment (FMA), modified Rankin (mRS) Functional Independence Measurement (FIM). Cognitive Montreal Assessment (MoCA), Frontal Battery (FAB), Trail-making Test A B tests (TMT). Patients Group received secondary prevention performed exercises complex for general muscle improvement 2 months; exercise paretic hand months.
 Results. After months significant increase FIM “Self-care”, “Transfer” therefore subtotal total scores observed 2; only scores. Adding (Group 2) appeared be more beneficial function. showed FMA “Wrist” score by 40 % (p<0.05), “Hand” 42.8 (p<0.01) “Total function” 30.1 10.9 higher comparison (p<0.05). In status differences compared baseline level MoCA increased 14.3 (p<0.05) TMT-B performance time decreased 14.8 Baseline correlated (r=0.32; p=0.028), (r=0.49; p=0.001) (r=0.46; p=0.004) correlation (r=-0.032; p=0.025), (-0.45; p=0.009), “Speed/coordination” (r=-0.023; p=0.036) (r=-0.42; p=0.023).
 Conclusion. into post-stroke contributes better is favourable patients’ independence. Upper impairment, especially wrist dysfunction, associated worse performance. Hand fine skills could cognition. Further research needed regard prognostic significance findings their treatment rehabilitation strategies.
